Food Permit Application - FOOD PERMIT APPLICATION

Your facility is required to maintain compliance with the St Clair County Health Center Food Ordinance. Part of this compliance is Food Class attendance by Shift Managers and Food Handlers who work to serve, prepare and handle food to the general public. The Food Workers are required to attend at least one class a year for compliance. You can determine which class is best suited for your schedule this year. You will need to RSVP to 417-646-8332, ask for Evelyn, so we will know how many food handlers will be attending from your facility. The cost is $10.00 per person. Your cooperation in maintaining a safe, sanitary, and healthy atmosphere is appreciated.

Well Water Testing
Private well testing through Missouri Department of Health and Senior Services. Pick up a water test kit. Ask for further information on testing instructions. All private water samples received at the MSPHL for bacteriological testing will require the normal $10.00 handling fee in order to be approved for testing.
