You may also obtain the application form at St. Clair County Health Center office. 530 Aduser Drive Osceola, MO 64776-6284

2. Complete the application form and bring it to the St. Clair County Health Center Offices Monday- Thursday
8:00am-4:15pm

3. Pay the appropriate application fee:

Birth Certificates $15.00/each

Death Certiciates

$14.00/first copy

$11.00/each additional copy
